Send completed forms to Patient Services, fax them to the office, or bring a printed copy to your appointment.
You can email the front and back of your photo ID and insurance cards with your New Patient Packet, or bring them with you to your appointment. Please also bring a referral if required by your insurance plan.

For your appointment with Anthony L. Cuppari, MD, FACS, please bring your photo ID, current insurance card, medication list, and any relevant medical records, test results, or imaging reports. If another physician referred you, please also bring any referral or authorization required by your insurance plan.
If your insurance plan requires a copayment, please be prepared to make your copayment at the time of your visit.
Referral requirements depend on your individual insurance plan. Some plans require a referral from your primary care physician before a general surgery consultation with Dr. Cuppari. We recommend checking with your insurance company before your appointment if you are unsure.
New patients seeing Dr. Cuppari should complete the Patient Information, Health History, and HIPAA forms before their first appointment. Completing your forms ahead of time can help make the check-in process quicker and easier.
Please email your completed New Patient Packet along with a clear copy or photo of your photo ID (such as a driver’s license) and the front and back of your insurance card to INFO@CUPPARIMD.COM.
Medical records, test results, referrals, and other requested information may be sent to our office by fax or the designated Patient Services email. Please include the patient’s name and date of birth so the information can be properly matched to the patient’s record.
